The collision happened on the A4 near the Hogarth roundabout on Friday, January 18 at around 8pm.
Eileen Daley, 58, from Shepherd's Bush, was struck by two vehicles travelling westbound - a blue Renault Clio and a London bus - on the westbound carriageway.
The drivers of both vehicles remained at the scene.
Next of kin have been informed and a post mortem has taken place.
PC John Roberts from the Met's Collision Investigation Unit at Hampton, said: "We are appealing for anyone who may have seen this incident to contact us.
"This is a major road and the collision took place at what would have been a busy time so if you were around the location at 8pm, please contact us."
