St George's Healthcare NHS Trust has launched a scheme to help young kidney disease patients receive hospital care.
Part of a national 18-month improvement plan, led by quality improvement agency NHS Kidney Care, a team of consultants, nurses, psychologists and social workers will work toward ensuring consistent care for patients aged between 17-25 as they move from paediatric to adult renal units.
There are plans for education and information programmes and a peer support network for young adult patients.
Joyce Popoola, consultant nephrologist at St George’s Hospital, said: “This group of patients have unique needs.
"Our aim is to make sure that these patients have the best support possible to manage their illness while going through major changes in their lives."
